Description

We are currently recruiting for an experienced Adult Community Occupational Therapist to join a well-established local authority team in London.

This is an excellent opportunity to work within a multidisciplinary community service, supporting adults to maximise their independence, promote wellbeing and enable them to remain safely within their own homes.

The successful candidate will manage a varied caseload, carrying out functional assessments, recommending equipment and adaptations, and working collaboratively with health, housing and social care professionals to achieve positive outcomes for service users.

Key Responsibilities:
- Manage a caseload of adults with a wide range of physical disabilities, long-term conditions and age-related needs within the community.

- Undertake comprehensive occupational therapy assessments in service users' homes and other community settings.

- Assess functional abilities and identify interventions that promote independence and reduce the need for ongoing care.

- Recommend and prescribe specialist equipment, minor adaptations and assistive technology.

- Complete moving and handling assessments and provide appropriate recommendations.
Assess for major adaptations, including Disabled Facilities Grant referrals where appropriate.

- Develop person-centred intervention plans that support safe discharge, independence and community participation.

- Work closely with social workers, physiotherapists, nurses, housing teams, carers and other partner agencies.
Undertake risk assessments and implement appropriate risk management plans.

- Maintain accurate clinical records, assessment reports and case documentation in accordance with professional and organisational standards.

- Participate in safeguarding procedures where concerns are identified.

- Provide advice and education to service users, carers and families regarding equipment, adaptations and strategies to maximise independence.

- Contribute to multidisciplinary meetings and service development initiatives.

- Ensure compliance with HCPC standards, professional guidelines and relevant legislation.

Essential Requirements:
- Degree in Occupational Therapy.
- Current HCPC registration.
- Previous experience working within Adult Community Occupational Therapy services.
- Experience carrying out functional assessments within community settings.
- Knowledge of moving and handling principles and equipment provision.
- Experience recommending minor and major adaptations.
- Excellent assessment, communication and report-writing skills.
- Ability to manage a busy caseload and prioritise effectively.
- Full UK driving licence and access to a vehicle may be advantageous for some roles.

Desirable Experience:
- Experience working within a London local authority.
- Experience completing Disabled Facilities Grant assessments.
- Experience using local authority case management systems.
- Knowledge of safeguarding adults procedures.
- Experience working within multidisciplinary community teams.
