Candidates need to have the following documentation prior to commencing work for PSL:

• An updated CV (no gaps in employment for the last 5 years - include travelling time)
• Two referees, preferably from your most recent employers
• An HCPC certificate
• Degree (undergraduate and post graduate certificate desirable)
• A Disclosure and Barring Services check formally called a Criminal Records Bureau Disclosure (CRB). Non UK / EU nationals would be advised to present an International Police Check should they have one.
• Passport
• Visa (as appropriate)
• Drivers licence and/or birth certificate
• Mandatory training certificates (Manual Handling / CPR / Online Training) Candidates are required to renew their training on an annual basis.
• Vaccination certificates (Hep B / Varicella / TB / Measles / Mumps / Rubella )
• Two Proof of address (bank statement and/or utility bill)
• CPD Certificates
• One passport picture

All documents need to be originals with vaccination reports either on the medical practices letter head or stamped and signed by the GP. Vaccinations that are presented in vaccination booklets are accepted.
